About the Author
Professor, PhD Petter Laake has since 1989 been a staff member of the Institute of Basic Medical Sciences, Department of Biostatistics, University of Oslo. He has 20 years of experience in lecturing, communicating and advising in statistics, at all levels and for various target groups. Since 2001, he has worked with the postgraduate studies programme at the Faculty of Medicine, University of Oslo, and has been in charge of the mandatory basic course in research methods. He has written and edited several text books in research methodology and statistics.

Professor, MD Haakon Breien Benestad has since 1968 been a staff member of the Institute of Basic Medical Sciences, Department of Physiology, University of Oslo. He has lectured students of medicine, odontology, nutrition and physiotherapy and has been an advisor for master and postgraduate students. He has held various basic courses for postgraduate students, compiled compendia for these courses and written a textbook of anatomy, physiology and immunology for social and health studies in upper secondary schools.

Professor, MD, Bjorn Reino Olsen is Hersey Professor of Cell Biology at Harvard Medical School and Professor of Developmental Biology, Harvard School of Dental Medicine. Since 2005, he also serves as Dean of Research at Harvard School of Dental Medicine. He has lectured worldwide and mentored and trained a large number of students and postdoctoral fellows. He is a member of and has held leadership positions in several professional organizations, has served on several Editorial Boards, and is currently Editor-in-Chief of Matrix Biology and BioMed Central’s Journal of Negative Results in Biomedicine.

About the Author
Guido W. Imbens is Professor of Economics at the Graduate School of Business, Stanford University. He has held tenured faculty positions at Harvard University, the University of California, Los Angeles, the University of California, Berkeley, and Stanford University. He is a fellow of the Econometric Society and the American Academy of Arts and Sciences. Imbens has published widely in economics and statistics journals, including Econometrica, The American Economic Review, the Annals of Statistics, the Journal of the American Statistical Association, Biometrika, and the Journal of the Royal Statistical Society.

Donald B. Rubin is John L. Loeb Professor of Statistics at Harvard University, where he has been professor since 1983 and department chair for thirteen of those years. He has authored or coauthored nearly four hundred publications (including ten books), has four joint patents, and has made important contributions to statistical theory and methodology, particularly in causal inference, design and analysis of experiments and sample surveys, treatment of missing data, and Bayesian data analysis. Rubin has received the Samuel S. Wilks Medal from the American Statistical Association, the Parzen Prize for Statistical Innovation, the Fisher Lectureship, and the George W. Snedecor Award from the Committee of Presidents of Statistical Societies. He was named Statistician of the Year by the American Statistical Association, Boston and Chicago chapters. He is one of the most highly cited authors in mathematics and economics with nearly 150,000 citations to date.

If you need something that teaches you both the concepts of mechanical ventilation and how to manage patients with respiratory failure, this is the book for you. I can remember my first ICU rotation as a resident. It was great--sick patients, lots of procedures, and a real challenge. It was also intimidating, especially when it came to the ventilator. I had a basic understanding of mechanical ventilation, but I wanted to learn more. Unfortunately, the books and articles that I found were not what I needed. Don’t get me wrong--there are a lot of excellent books on Critical Care Medicine out there. The ones that I read had long chapters on the pathophysiology of respiratory failure, the mechanics of gas exchange, and the evidence supporting different modes. The problem was, I didn’t have time to read 100 pages on the mechanics of gas exchange. The physiology stuff was nice, but it wasn’t much help in the middle of the night when I was getting paged to the ICU. What I needed was a guidebook--something that would tell me how to set the ventilator up, explain why, and help me troubleshoot a tough situation. It had to fit in my jacket pocket. It had to be written in a way that made sense. Most importantly, I had to be able to read it at 2 a.m. without having it put me to sleep! Now, I teach Critical Care Medicine. The residents and medical students that I work with need the same thing I did--a way to understand mechanical ventilation while being responsible for the care of sick, sick patients. That’s why I wrote The Ventilator Book. So...what’s in the book? The How-To Guide--here’s where you’ll find good information about initial setup, quick adjustments, and troubleshooting. The How-To Guide is all you need to get through a busy night on call in the ICU. This is available for free on the website. The Eleven Commandments of Mechanical Ventilation The Owner’s Manual--this is a more in-depth discussion of different modes, PEEP, trigger, flow, and liberation from mechanical ventilation. There are also chapters on high frequency oscillatory ventilation and airway pressure release ventilation, as well as a chapter on taking care of the patient with prolonged respiratory failure. Each chapter is concise and can be read in 10-20 minutes. Appendix of Useful Knowledge--equations and formulas that are useful for attending rounds, pimping, and presentations. They can also be used from time to time to take care of critically ill patients. This is available for free on the website.

About the Author
I am a graduate of The Citadel (Echo ’96) and the University of South Carolina School of Medicine. I completed my residency training in Emergency Medicine at the Earl K. Long Medical Center in Baton Rouge, Louisiana. I then did a fellowship in Critical Care Medicine at Tampa General Hospital in Tampa, Florida. I am board-certified in Emergency Medicine, Critical Care Medicine, and Neurocritical Care. Currently, I am an Assistant Professor of Critical Care Medicine at the University of South Carolina. My interests include mechanical ventilation (obviously), neurocritical care, ultrasonography, and the rational clinical examination. My passion is teaching others the craft of caring for the most desperately ill and injured patients. Outside the hospital, I am a proud husband and a father of three children and two St. Bernards. I like beekeeping, homebrewing, whitewater kayaking, rock climbing, and SEC football.

What you need as a resident in the ICU!!

I am a PGY3 Internal Medicine resident. Through my residency so far I had been reading about ventilator management from so many different source that all the information gets mangled. You are reading the same stuff from different books and thus you fail to develop a mental picture. It is always good to have a single basic source. This is an excellent book for anyone wanting to know the basics and more for ventilator management. I am reading it on my Note 3 which has a big enough screen size to read books. I am reading it on the go when I have time. It is short enough and the language is simple enough for it to be read and applied quickly. It gives just the required bit of knowledge to apply to your patients which is I think all you need. Not extensive but all a resident would need and also if you are planning for a career as a Hospitalist who would have to do Vent Management.

Read this one or twice and that should set you up.
I would recommend this for Residents and Hospitalists and possibly critical care fellows as a great starter book.

Light, Quick, Great to read

Page 65-66 Had me laughing out loud on rounds when I was sneaking in some reading time. - Anyway, I just don't have time to read about the pathophysiology and the mechanics on breathing they way in an ideal world would exist in. (1)There is seriously no time to whip out a textbook every night managing ICU patients when you start out. (2) When speaking to my attendings, even when I was wrong, my application was sound, and to me that's a huge point with the attendings and medicine overall. Residency is a learning experience, the more you man up justifying your reasoning with conceptual thinking, everyone considers it, more than belting out facts. The Book helped immensely in "thinking it out" on the fly. The way it is written is very conceptual. Honestly, there is no time to be relying on anything but "muscle memory."

Concise, Clear and adequate.

This is one of the best books I have ever read on ventilators. It's like a running commentary. It's consise, clear and full of realistic examples introduced at the right time (just before the concepts make you to start scratching your head). Of course as Dr. Owens admits, there are many more detailed books on mechanical ventilation which you can read for more knowledge. This book is so "down to earth" that any beginner can make sense out of it and any expert would agree with what I just wrote above. Congratulations Dr. Owens! Please write more books.
